Foreword to the French edition, by Dr. Blaise Rutschmann and Dr. Christophe Perruchoud, founders of the Pain Clinic at the La Tour Hospital (Meyrin, Switzerland)
"The reality is obvious: musculoskeletal disorders are omnipresent. And, to put it another way, who has never had pain, especially in the back? A pain clinic is dedicated to the prevention and treatment of chronic pain. Chronic pain is defined as pain that has lasted for more than 3 to 6 months. Half of all patients who visit a pain clinic worldwide do so because of spinal pain, whether in the lumbar, cervical or thoracic region. Patients come to us because their condition has had a first episode, then often a second and then multiple episodes, the frequency of which only increases with age. Contrary to popular belief, it is not only people who perform heavy work, or forced laborers who are affected. And it's not only the elderly who suffer from back pain. It's "Mr. and Mrs. Everybody", and at any age. Our lifestyles have changed, evolved. We see that the younger generations are extremely concerned; life on computers, tablets or smartphones is certainly not foreign to it. Who does not own or work, at least partially, on a computer? Who has never booked their flight for their next vacation on a smartphone? Who doesn't communicate daily with friends, children and grandchildren via e-mail? Who does not read, play or look for information on these tiny screens, installed in positions that our parents or grandparents did not know? If the origin of back pain often remains difficult to objectify, their reality is a fact. To designate on the basis of multiple complementary examinations the place, the joint or the disc at the origin of the pains remains an ink bottle, with multiple traps. Indeed, apart from well-defined and well-known syndromes, the results of complementary radio examinations (xray, magnetic resonance imaging (MRI) examinations, for example) are often ambiguous, difficult to correlate with the patient's clinical examination and do not necessarily make it possible to definitively orient treatments. Moreover, drug treatments, physiotherapy, potential infiltration in specialized centers or surgical management, minimally or maximally invasive, are reserved for specific cases. This is where Olivier Girard's book takes on its full meaning. It is clearly established that the patient is today at the center of his treatment. Recent major medical publications confirm this (reference: The Lancet, Vol. 391, No. 10137, published: March 21, 2018, series of three articles). This in no way contradicts the medical care provided by a primary care physician, a specialist or a center dedicated to the treatment of rheumatological, neurological, orthopedic or chronic pain conditions. Multidisciplinary care is not just a fad, it must become a matter of course. But without the patient's participation, without his active involvement, everything is much more difficult. The analysis of posture, of imbalances of the spine in the three planes of space is a discipline in full expansion, as much on the ergonomic level as on the orthopedic level. Olivier Girard's book is therefore very timely [...]. Posture is always built from bottom to top (isn't that logical?), the neck should always be as relaxed as possible, whether you are driving, pregnant, sitting or sitting down, at work or on public transport. Back pain is not a fatality. In primary prevention of their occurrence, in secondary prevention after a first or multiple episodes, there is always time to consider changes, clearly motivated and outlined as suggested in the book you have in your hands. Since each person is the best asset in his or her own care, it is time to start reading it, which we know will be enjoyable, and which we hope will allow you, thanks to the teaching of a great professional, to obtain quick and above all lasting results." |
